Dump all the ingredients (except the Sprinkles) in the crockpot. Cover with the lid and slow cook for 2 hours on Low or 1 hour on high. Stir every 20 minutes to avoid the chocolate from sticking to the bottom of the pot and burning.
Once the chocolate gets melted, spoon a tablespoon of the mixture onto a parchment paper lined baking dish. You can use a cookie scooper or melon scooper as well for this step. Or just use a spoon and a fork. Top the candies with Christmas sprinkles and freeze for 5 minutes.
Serve or store in an airtight container for future.
